Please complete this FORM to request that school records be forwarded to an outside entity.  State law requires that pupil records cannot be released without the written consent of a parent, guardian, or adult student.  A Notarized Authorization for Release of Records Affidavit must be on file at GCA.  **It takes 7-10 days for records /transcripts to be sent to the receiving school district**

Georgia Cyber Academy High School Student Transcript Request/Release Form for Sending to Multiple Colleges– 
Please complete this FORM to request that school records be forwarded to multiple colleges.  State law requires that pupil records cannot be released without the written consent of a parent, guardian, or adult student.  A Notarized Authorization for Release of Records Affidavit must be on file at GCA.

Resources:

  • eADAP – By law, Georgia teens under the age of 18 are required to successfully complete the Alcohol and Drug Awareness Program (ADAP) in order to obtain their Class D Georgia driver’s license.  GCA students can complete the eADAP Program online.    

  • Get Registered! – Students can now self-register for eADAP at https://online.dds.ga.gov/eadap/StudentRegistration
